Hi JDWoods,

There is a contact form already built in to the core of Joomla 2.5. Look under Components > Contact Manager, and you can customize it.

Is that what you are looking to do?

Or you can just install a contact form extension such as this:
extensions.joomla.org/extensions/contact...k/contact-forms/8219

Seems to have a high rating and simple to install and use.
